New Trier Township offers grants to those with disabilities

New Trier Township Government is accepting grant applications from eligible residents for the Community Support Grants Program for Persons with Disabilities. This program, launched in 2007, provides financial assistance to individuals with disabilities to help with some of their special service needs.
The goal of this “Money Follows the Person” program is to promote self-direction (or consumer direction) of services,” notes Patricia B. Cantor, Supervisor. “In the five years since inception it has provided greater choice and control over the services received by Township residents with disabilities and their families in their preferred setting,” she adds.
Applicants must be residents of New Trier Township and qualify with a condition listed in the DSM-IV diagnostic code. There are no income or age restrictions. A list of eligibility considerations, services covered by the program, and application can be obtained by calling the Township office at 847-446-8202, visiting the Township Office at 739 Elm Street in Winnetka or downloading the forms from the Township website http://www.newtriertownship.com/services/community_support_grants_program.aspx
Applications will be due on Friday, December 30, 2011. The Township will select as many individual and family participants as funding allows in a random drawing. Selected recipients will be notified by January 31, 2012.
